Free or low-cost Insurance &
Medical Coverage Programs for Children

For local information and applications, contact 751-8110. Completion of a universal application will determine eligibility for: Montana Medicaid, Children's Health Insurance Program (CHIP), Caring Program for Children, Special Health Services, and Montana Mental Health Services. An online application & additional information is available at: www.dphhs.state.mt.us/hpsd/chip.htm or www.insurekidsnow.org (must click on CHIP)

PROGRAMS BASED ON INCOME


MEDICAID Provides medical coverage to children, based on family's income. Takes into consideration family assets and resources. Family is not eligible for coverage if countable resources and assets exceed $3,000 (family can exclude 1 house and 1 car).
CHIP This is a low-cost, private health insurance plan that provides coverage to low-income children through age 18. Financial eligibility is based on family's adjusted income and household size. Doesn't count family assets or resources: savings, IRA's or extra vehicles.
CARING PROGRAM FOR CHILDREN Primary and health care services for children who aren't eligible for any public health care plan including Medicaid, Medicare, or any other government-sponsored program.

PROGRAMS BASED ON INCOME & PHYSICAL CONDITIONS

SPECIAL HEALTH SERVICES Assists families with medical costs that arise from treating children with special health care needs including, but not limited to: heart conditions, cleft lip, seizures, asthma, diabetes and concerns with child's development.
MHSP: MENTAL HEALTH SERVICE PLAN Provides coverage for individuals with severe disabling mental illnesses and serious emotional disturbances who need counseling, therapy and medication.

PROGRAMS BASED ON PHYSICAL CONDITIONS ONLY

MCHA: MONTANA COMPREHENSIVE HEALTH ASSOC. Provides comprehensive health insurance benefits to high-risk individuals. Includes, but not limited to: coronary artery disease, leukemia AIDS, malignant tumor, and lupus. Also covers people who have been rejected by two insurers within the last 6 months

LOW COST INSURANCE OPTIONS

MONTANA YOUTH CARE A private health insurance plan that provides a child with comprehensive coverage. Preventive care is covered.
NORTHWESTERN SCHOLASTIC INSURERS Is a private health insurance plan that provides a child is in school with supplemental accidental bodily injury coverage.

LOCAL CLINICS

SHEPHERD'S HAND CLINIC First come- first seen basis. Monday evenings 6:00- 10:00 p.m. Non-emergency medical care.
Christ Lutheran Church 233 Kalispell Avenue Whitefish
TOUCH OF GRACE FREE MEDICAL CLINIC Non-emergency medical care, Tuesday nights starting at 6:00 p.m. Call for appointment 8am-5pm, M-F @ 257-4357. May help with prescriptions.
Located at Salvation Army 110 Bountiful Drive Kalispell
FLATHEAD CITY-COUNTY HEALTH DEPT. Offers health services in the following areas: Immunization Clinics, Low Birth Weight Prevention (MIAMI), WIC (nutrition), Reproductive Health Services, Tobacco Cessation, and Children's Safety Pro
grams. Located at 1035 1st Avenue West.
