Flathead City-County Health Department
offers two home visiting programs, MIAMI and Follow Me. Home visiting provides an opportunity
to meet the family on their own ground, giving the family a better
sense of control and involvement in identifying and meeting their
own health needs. For the nurse, visiting in the home provides a
much clearer picture of family strengths and needs in an informal,
relaxed setting in which the nurse is a guest in the family home.
MIAMI Project (Montana's Initiative for the
Abatement of Mortality in Infants) is a statewide
low birth weight prevention project that works with pregnant women
and their health care providers to help them have a healthy full-term
pregnancy. Services available through this project include: Case
Management which includes assessment and referrals by Nurses, Social
Workers and Dieticians; Pregnancy, Labor and Delivery, and Parenting
information and education.
Medicaid presumptive eligibility is a service provided to assist pregnant women in accessing Medicaid early in their pregnancy thereby helping
them to access prenatal care early.
Follow Me Program begins at birth up to the age of 5. Public Health nurses provide screenings, parenting information, referrals and
anticipatory guidance. The goal of this project is to enhance child growth and development, promote parenting and ensure early intervention services
as needed so all Montana children will enter school healthy and ready to succeed.
