Billing Information at Flathead Community Health Center

• We accept all major insurances, as well as Medicaid, Medicare, Workman's Compensation, and Tricare.  Please present your insurance card at check-in.

• We have a sliding fee scale to calculate discounts for individuals requesting reduced fee.  Please bring proof of income (recent pay stubs and last year's income tax form.)

• We do expect payment at the time of services.

• We calculate discounts using Federal Poverty Level guidelines and household income.

• Charges are discounted on a sliding scale for those at or below 200% of Federal Poverty level (example: an individual earning 20k per year.)

• Minimum fees are charged for those with household income at or below the Poverty level (example: an individual earning 10K) are charged.

• The minimum fee for an office visit is $10.

• There will be additional charges for lab tests (pregnancy test, strep test, flu test, etc) or office procedures (mole removal, wart freezing, wound repair.)

• We are a non-profit organization and welcome donations.
